Continued care management is important to Premier Senior Care. We understand that the needs of seniors change with the inevitable effects of aging. This is why Premier Senior Care continues to assess the needs of their clients and make revisions to the Care Plan as needed. Caregiver Selection

Careful selection is made for a home caregiver to match the seniors needs & personality.  If a senior has Alzheimer’s, only a caregiver who has experience with Alzheimer’s care will be selected. All our caregivers are carefully screened.  Learn more about our reliable home caregivers.

Hands On Orientation

The selected caregiver is introduced to the senior and provided with the home care plan that was customized for your loved one. The caregiver participates in a hands-on orientation to guarantee that the seniors needs will be take care of in full.

We Manage

Your assigned Care Manager will continue to monitor the care of the senior.  We continue to oversee the caregiver and maintain constant communication to make sure that the care is continuing to meet the needs of your loved one

Continued Assessment & Care Planning

As the needs change for the senior, the Care Manager will make assessments and recommendations to you and  your family. If revisions are necessary to the seniors care then changes will be made to the plan of care and thoroughly communicated to the caregiver.
